北京协和医院肝脏外科圆满完成CMB第一个财政年度工作

        北京协和医院肝脏外科于日前向美国中华医学基金会(China Medical Board of New York,CMB)提交了第一个财政年度的进展报告。报告已交到CMB在纽约的总部,我们的工作得到了总部的肯定和赞许。


        肝脏外科在第一财政年度里圆满完成了CMB计划中所要求的任务,工作包括CLTTC的构建,专业人员的培训工作,国际、国内专家的交流,CMB专业资助的会议,中心所在单位研究工作及国内外的研究合作工作,以及帮助推动国内肝移植相关的法制法规的建设。在其中人员培训、国际合作和帮助推动法制法规方面,肝脏外科超额完成了任务,尤其在法制法规方面得到了CMB主席L.chen的高度肯定。


        紧接着第二财政年度的工作已在紧锣密鼓地开展之中。

VI. The Narrative

 

According to the planned CMB project, we have performed the following programs within this fiscal year.

Having set up basic frame of CLTTC

We have finished the basic organization of CLTTC. The Center’s brass plates have been formally put up at Peking Union Medical College Hospital (PUMCH), Tianjin First Central Hospital Transplantation Center, and San Yat-Sen University Transplantation Center. The three units of the Center have kept with well cooperation. In early of the year, we have arranged a special office and employed a special full-time secretary for CMB program in PUMCH. After that in January, we have established own lab center sponsored by CMB.

In March 2007, two of our major staffs visited University of Chicago School of Medicine transplant section; exchanged the experiences, increased mutual understanding, and drew up a cooperation plan upon CMB project. Professor Sang and Professor Mao have joined the conference regarding the transplantation ethical issues there in Chicago, and had discussions on the comparison of ethical issues in both the US and China sides. Professor Mao has also given a speech regarding the research and clinical works of Liver Surgery Department, PUMC at a Surgical Grand Round in Department of Surgery, University of Chicago.

 

Having starting theoretical and clinical training of liver transplantation

 

  We have held "07 ′liver surgery/liver transplantation new technique training program" in Beijing on April 20-23, 2007. This program is sponsored by CMB. We have invited 12 experts from the United States, Europe, Hong Kong, and mainland Chinese, relatively. Total recruited trainees were 120. Lectures involved the basic theory and advanced progress of liver transplantation, such as treatment of peri-operation, skills of operation for liver transplantation, treatment of complications, and application of immunosuppressive agents. Trainees have got opportunity of participating in an exchange of experience each other, and with experts. They also have chance to apply cooperation programs for CMB and possibilities of obtaining proper financial support. In addition, 47 trainees have visited the one of two partners of this CMB project, TianJin First Center Hospital Transplantation Center, the biggest transplant center in China.

 

Developing research tasks

 

          We have coordinated scientific research works regarding liver failure, liver cancer, and liver transplantation with some well-reputation institutes or centers. The researches has going on fairly well. These researches include molecular biology and gene, the mechanisms of hepatic failure, the mechanisms of immuno-reaction and rejection, immunosuppressive agent, and grafts recovery mechanism. At this time, at least three papers regarding the above research areas have been published in the first-ranked medical journals in China. We are presently perfecting the lab facilities gradually, and carrying out the various research projects.

 

Interchanging programs and relative conferences

 

The National Clinical Application of Human Organ Transplantation Summit, partially sponsored by CMB, has been held by the Ministry of Health and Sun Yat-Sen University transplantation center in Guangzhou on November 13-15, 2006. It was the first human organ transplantation summit convened by the government authorities. It gathered together more than 600 administrative officers and clinical organ transplant professionals. The 2nd Meeting of Ministry of Health Committee on Clinical Application of Human Organ Transplantation was also performed during the summit. It performed the following contents: a) announcing the approval system on human organ transplantation; b) helping expert committee to formulate approving norms on clinical application of human organ transplantation. The summit had invited some experts from the countries and areas including the United States, UK, Hong Kong, and Taiwan to introduce their experience on organ transplantation, and had also carried out unveiling ceremony of CLTTC for three centers (Beijing, Tianjing, and Guangzhou).

 

Helping establishing administrative and registration system of liver transplantation, and pushing forward the establishment of relative law and legal rules.

 

For the purpose of regulating the clinical application of the human organ transplantation, ensuring the quality and safety of medical services and protecting the health of patients, sponsored by CMB, the Ministry of Health launched “Interim Provision on the Administration of Clinical Application of Human Organ Transplant Techniques” on July 2006, and promulgated “Norms on Administration of liver transplantation technique” depending on the provision. “The 2nd Meeting of Ministry of Health Committee on Clinical Application of Human Organ Transplantation and National Clinical Application of Human Organ Transplantation Summit Declaration” was announced in Guangzhou on November, 2006. The declaration called for domestic medical and other fields to obey medical standard and ethics principle, to abide by occupation morals, to do transplant operation on the basis of law regulation, to prevent organ trafficking and transplant tourism.

After extensively discussed, sponsored by CMB, “Human Organ Transplantation Act” has formally issued by State Council in April 2007 and in act on the 1st of May 2007.

National Organ Transplantation Committee (OCT) of the Ministry of Health has also called relative experts for the first time to deliberate the qualification for domestic organ transplantation institutions and personals. The first batch of the institutions matching the norms of liver transplantation will be only 150 in number, dropped from over 600 across the nation. This achievement was also supported by CMB.

The starting model of the national registration system for the organ/liver transplantation has been set up in University of Hong Kong. This system is computer based, and now there are 21 transplantation centers domestically joined the system. If the model runs well, it will be expended to the nationwide, and moved into mainland China.

 

VII. Significant Achievements:

 

The most significant achievements have been, in the short period of time, to establish the rules and relative act for organ transplantation in China, set up the qualification for the transplant for medical institutes and specialists, restrict the number of transplantation units from over 600 to 150. At the mean time, the Center has trained some 120 trainees in the first fiscal year, which has over fulfilled the schedule of the project. These works were sponsored or partially sponsored by CMB. 

Apart from that, a professional website of our own Department has been built on the most influenced biological medical forum “Din Xing Yuan”. In the beginning of the website establishment, an announcement of "an open letter to the all domestic surgeons of the liver surgery" was made to introduce the CMB project, and let more people understand the event.

 

VIII. Significant Problems:

 

There are still numbers of problems existed that has become obstacles of the progress of liver transplant and has to be solved as soon as possible. Firstly, there is limited deceased donor pool. Secondly, it lacks a well-organized and performed administrative system responsible for national organ transplant registration, grafts sharing, allocation and implementation of national standard and protocol for indications and peri-transplant management, which lead to waste of precious resources and unsatisfactory outcome. Thirdly, the imperfection of legislation on liver transplant is the main restrictive factor for the healthy development of liver transplant. Lastly, liver transplantation remains one of the most expensive medical and surgical procedures performed in China. We will help establishing technical guidelines and requirement for qualification of liver transplant programs, setting up registry system and organ sharing system for liver transplant, speeding establishment of legal frames for brain death and organ transplant and engaging China’s liver transplant community into global community through international exchange programs.
